If a seizure occurs, clinicians should check clozapine levels and evaluate for triggers such as dose increases or kinetic changes. Divalproex is generally preferred if an anticonvulsant is needed, while several agents are advised against because of interactions or adverse effects. Clozapine plus ECT is presented as a safe and effective option for inadequate response.<br /><br />Cardiac and renal inflammatory reactions are highlighted as early-treatment risks. Fever is relatively common and often benign, but myocarditis should be suspected in the first 6 weeks when fever, malaise, chest pain, tachycardia, or dyspnea occur. Troponin and CRP are the key tests. Interstitial nephritis is also an early event and is evaluated with creatinine/eGFR and urinalysis. Cardiomyopathy typically occurs later and is assessed with echocardiography.<br /><br />The talk also covers less common hematologic abnormalities, including leukocytosis, neutrophilia, thrombocytopenia, thrombocytosis, and anemia; most are transient, though anemia may require broader work-up. Enuresis is common, underreported, and may improve with slower titration, dose splitting, and behavioral steps.<br /><br />Finally, inadequate clozapine response is often due to nonadherence or underdosing. Management includes maximizing clozapine exposure when tolerated and considering adjunctive antipsychotics or ECT.
